DAY HILL DOME PARTNERS, LLC (dba DAY HILL DOME) I am aware of the nature of any sports activity, and I hereby assume responsibility for myself or my child to participate. I will not hold Day Hill Dome Partners, LLC, and/ or its employees or agents responsible in case of any accident or injury as a result of this participation. There will be no refunds issued. I hereby further agree to indemnify and hold harmless Day Hill Dome Partners, LLC, from and against any and all loss, damage, claim, demand, liability, or expense by reason of any damage or injury to property or person which may be claimed to have arisen as a result of or in connection with participating in activities at the Day Hill Dome. In addition, I grant Day Hill Dome Partners, LLC, and its staff permission to utilize any medical emergency services it deems necessary to treat injuries that I or my child may incur while participating. I understand that Day Hill Dome Partners, LLC, does not provide insurance for program participants. I agree, understand, and acknowledge, on my own behalf that an inherent risk of exposure to the disease COVID-19 (as defined by the World Health Organization and any strains, variants, or mutations thereof) and SARS-CoV-2 (the virus that can cause COVID-19) (collectively, “COVID-19”), and any other communicable or infectious disease, exists in any public place where people are present. Day Hill Dome Partners, LLC may be taking pictures of activities to use in promotional publications. I hereby release and permit the Day Hill Dome Partners, LLC to utilize these photos for Day Hill Dome promotional purposes.
